1. General Questions
1.1 What is GoToMyClinic?
GoToMyClinic is a website you can use to view your health information securely. GoToMyClinic is available 24/7 and can be accessed from any computer with an internet connection. It can help you answer questions such as, “What are my lab results? Which doctor treated me during my stay?” And more!
1.2 Who is eligible for GoToMyClinic?
If you are 18 years of age or older and have been a patient at the family medicine Apple Valley Medical Clinic, you are eligible to enroll in GoToMyClinic.
1.3 Is GoToMyClinic secure?
Yes. GoToMyClinic is a secure website. We have put numerous checkpoints in place to ensure that you are the only one who can view your health information in GoToMyClinic. You are required to set up a user name and password before you can log in. In addition, all of your health information is encrypted, which means you are the only one who can view it, and no health information is saved on your computer.
1.4 Is there a fee to use GoToMyClinic?
No. GoToMyClinic is a complimentary service provided by the Apple Valley Medical Clinic.
1.5 How do I log into GoToMyClinic?
To log into GoToMyClinic:
- Click on the Login link in the menu bar of www.applevalleymedicalcenter.com.
- The Login window displays.
- Enter your Portal user name and password in the fields provided.
- Click the Login button.
1.6 Can I look at my children’s records in GoToMyClinic?
No. Currently, health information for minors (individuals under the age of 18) is not available in GoToMyClinic. If you are interested in obtaining your child’s health information, please contact our Health Information Management (HIM) department.
1.7 Can I look at my parent’s medical records in GoToMyClinic?
No. However, your parent can request access to GoToMyClinic and share his or her health information with you.
1.8 Can I deactivate my GoToMyClinic account?
Yes. You can deactivate your GoToMyClinic account any time you wish. Please come to the clinic and sign a form stating you want to deactivate your portal account.
2. Enrolling in GoToMyClinic
2.1 How do I enroll in GoToMyClinic?
Before you enroll in GoToMyClinic, we verify your identity and e-mail address. Once you have been treated at the family practice clinic, you will be eligible to enroll. Shortly after your visit, you will receive an e-mail with information on how to enroll. Please follow the steps in this e-mail, and if you have any questions, just refer to the manual on the home page. After you have enrolled, you can access the patient portal through www.applevalleymedicalcenter.com.
2.2 How do I choose my personal GoToMyClinic password?
After clicking on the link provided in your e-mail from GoToMyClinic, you will be prompted to set up your unique password. You will need to select a password that has at least six alphanumeric characters, which should include at least one number.
2.3 How does the password reset question and answer work?
While setting up your GoToMyClinic account, you will be asked to set up one question with an answer that only you should know.If you forget your password, you will be asked to answer this question before resetting your password.
2.4 What if I forget my GoToMyClinic username?
If you forget your user name, please call 952-432-6161, and your request will be forwarded to our Information Technology department. Someone will respond via e-mail within two business days.
3.1 What do I do if I am having a technical issue with GoToMyClinic?
If you are having a technical issue with GoToMyClinic, please call 952-432-6161, and our Information Technology support team will respond within two business days.
3.2 What if I have a question about the health information I am seeing in GoToMyClinic?
If you have a question about your health information in GoToMyClinic, please read the FAQ first. If that does not clear up the issue, then contact the clinic.
3.3 What if I need to update the information I am seeing in GoToMyClinic?
If you want to make changes to your information in GoToMyClinic, please call the clinic at 952-432-6161. Edited information will not be reflected in your records until approved by the clinic.
3.4 Where do I direct questions related to billing, insurance and scheduling appointments?
Please direct questions related to billing and insurance to the Business Office directly at 952-953-9160. Currently, we are not accepting any online appointments. For questions related to scheduling, please contact the clinic directly at 952-432-6161.
3.5 I received a phone call asking for my GoToMyClinic username and password. What do I do?
Our team will never contact you for your username or password. If you received a phone call from anyone asking for your GoToMyClinic username or password, please contact the clinic immediately at 952-432-6161. Do NOT give this information to anyone over the phone.
3.6 I e-mailed the GoToMyClinic support team four days ago and still have not heard anything. What is going on?
Please check your junk e-mail folder for response from the GoToMyClinic. If a response is not in your junk folder, call the clinic directly.
4. Medical Information in GoToMyClinic
4.1 Why am I not able to see my health information from other hospital systems?
At this time, you can only view your health information from the Apple Valley Medical Clinic in GoToMyClinic.
4.2 What information is available in GoToMyClinic?
Currently, you can access your current health conditions, including allergies, immunizations, medications and medication history; results of lab tests and any request for referrals. You also can print or save an electronic copy of your health summary or last office visit using the standard record and document formats. Currently, GoToMyClinic only displays health information from the time of the Patient Portal creation. Therefore, only health information from 2013 to the present is viewable in GoToMyClinic.
5. Lab Results
5.1 When will my lab results be available in GoToMyClinic?
If your lab test was completed during your visit at our clinic, it will be available in GoToMyClinic as soon as the doctor releases it. If your lab test was completed less than 72 hours ago, it may not appear in GoToMyClinic yet. After 96 hours, most lab results should be viewable. If you do not see your lab results after 96 hours, please call the clinic at 952-432-6161.
5.2 Where can I view my lab results in GoToMyClinic?
When the message first arrives, it will be in the My Recent Messages and My Recent Results sections on the Overview tab. On the Messages tab, the message will be included in the Inbox section with all other messages. On the Lab/Test Results tab, the message will be in the My Test Results section.
The messages containing lab or test results may include attachments. The paperclip symbol appearing to the left of the message date indicates an attachment.To view these results, click the title of the attachment.
5.3 What should I do if I have questions or concerns about a test result?
If you have any questions or concerns about your test result, please contact the clinic.
6. My Personal Information
6.1 If some of my health information in GoToMyClinic is not correct, what should I do?
If you want to make changes to the information you are seeing in GoToMyClinic, please call the clinic at 952-432-6161. Edited information will not be reflected in your records until approved by the clinic.
6.2 How can I update my telephone number, address or e-mail address in GoToMyClinic?
If you want to make changes to your telephone number, go to My Account at the top of the page and submit the changes through the portal. Edited information will not be reflected in your records until approved by the clinic.
6.3 I am looking for my medical information from 2010 but I am not seeing it. Where is it?
Currently, GoToMyClinic only displays health information from the time of the Patient Portal creation. This means you will only see health information from 2013. If you need information prior to 2013, please contact AVMC Health Information Management at 952-997-9850, ext. 4248.
7. Computer Requirements
7.1 What do I need to access GoToMyClinic?
Minimally, you need a computer with an internet connection and an internet browser, such as Internet Explorer, Firefox, Google Chrome or Safari. You can view GoToMyClinic using Internet Explorer versions 6, 7 or 8, but for the optimal experience, we recommend using version 9 and above.
7.2 What is the recommended screen resolution?
The recommended screen resolution is 1024 by 768 or above.
7.3 I do not have an e-mail address. How do I get one?
