How do I select a physician? 

Physicians who practice medicine at the Apple Valley Medical Center are located right here on our website.  You can see a photo of the physician, as well as background information and a short bio to help you make a decision.


Will I always see the same physician? 

We always try to schedule your appointments with the same physician, but there may be times when your physician is not available. If that happens, we will schedule you with another one of our excellent doctors.


How do I make an appointment?

Here are the various numbers you need to make an appointment.


What do I need to do before my first appointment?

When calling to schedule your first appointment, you will be asked for your name and address, date of birth and insurance information. Please have this information ready before you call.


What insurance do you accept?

We accept all major insurance plans. It depends on your insurance plan if you are in-network or out-of-network. Please call your insurance company to verify. If you have any questions about your coverage, please contact our office.


How do I transfer medical records from my previous physician?

Please contact your previous physician and ask that the forms be transferred to us. You will be asked to sign an authorization request before any records will be released.


What will happen at registration?

If you haven’t done so already, you will be asked to fill out a patient information sheet, a medical history form and a medication list. If you have medical records, x-rays or scans from another physician, please bring those with you. Also, bring your current insurance cards.

At registration for our family medicine clinic, you will be asked for your e-mail address. Once your registration is complete, you will receive an invitation to “My Patient Portal.” Signing up for the patient portal will give you online access to your health care information. You also will begin to receive our quarterly eNewsletter.


Do you offer urgent care services?

Yes. Our Urgent Care is open 365 days a year. There is always a physician on site, and you can receive lab and x-rays right here. Learn more about our Urgent Care here.


Do I need an appointment for urgent care? 

No. Patients are seen in order of registration, unless our staff deem it to be more urgent.


Do you provide imaging services? 

Yes. Through United Medical Imaging, we offer ultrasound, CT scans, MRIs and digital mammography.


What if I get sick after clinic hours or in the middle of the night?

Just call our clinic, and our answering service will relay your information to the physician on call. He or she will get back to you as soon as possible. You also may visit our Urgent Care.


How do I set up a lab appointment?

Most lab work needs to have a physician’s authorization before we can perform any test. Your doctor will tell you when you need to schedule these tests. We ask that you call in advance to schedule lab appointments to avoid long waits in the lab.


Can I call the clinic to ask my doctor a question?

Of course. Our receptionist will transfer your call to your doctor’s assistant. If your provider is not in the office that day, your call will go to someone else in the clinic, who will get back to you with an answer to your question.


How do I refill my prescriptions?

The fastest way is to call the pharmacy where your prescription was filled. Please allow a few days for the refill. If the prescription does not have any refills, the pharmacy will contact the clinic for authorization. We have a pharmacy on-site to make obtaining prescriptions convenient for you.


Do you have specialists on-site?

Yes. Through the United Specialty Center, we provide physicians in 14 different specialties, including ob/gyn, orthopedics and cardiology.


How do I see a specialist?

Your insurance will determine whether your primary care physician needs to make a referral or not. If so, your doctor will contact the specialist and send any records needed for the appointment. If your insurance allows you to contact a specialist on your own, you may set up an appointment directly with the specialist needed.


What if I need to be hospitalized? Where would I go?

Our physicians can admit you to any hospital you choose, although we have a strong working relationship with United Hospital in St. Paul. You may see a different physician during your hospital stay, but the physician who sees you will keep your doctor informed of your condition.


How do I pay my bill?

After we have filed any applicable insurance on your behalf, you will receive a bill from us. You may pay in person, by mail or by using our online bill paying function.


What if I have questions about my billing? 

Please call our main number at 952-432-6161, and someone will direct you to the appropriate billing number, depending on which medical practice you are using.
